Birthday Annual Review

From April 2014 we will be changing the way we manage patients with long term health conditions.      Patients who have conditions such as diabetes, asthma, COPD, hypothyroidism, cardiovascular disease or high blood pressure will have a single review for all their long term medical problems each year in the month of their birthday .  This will reduce the number of visits patients have to make to have their long term conditions monitored which we hope will be much more convenient.  We would encourage you to contact the practice around the time of your birthday to book a Birthday Review  appointment if this applies to you.

Transferring from the old system to a birthday review system is going to be challenging and may result in some patients being seen earlier or later than they initially expected.  The doctors would like to thank all their patients for their support whilst we are implementing these changes



Nurse Practitioners

Mrs Alison Williams (f)


Nurse Sue Johnstone
Sister Sue Johnstone has been with the practice since March 1990. She specialises in diabetes care, coronary heart disease, family planning, and family planning.
Nurse Emma Gulliver (f)

Emma Gulliver joined the practice in 2005 and is a generalist nurse dealing with immunisations, asthma, family planning, hypertension, anticoagulation, epilepsy reviews, smears and dressings.

Nurse Diane Chamberlain (f)
Qualified nurse Diane joined the team in August 2009 as a treatment room nurse.
Mrs Melissa Hocking-Carter (f)

Melissa joined the practice team in December 2015 having worked as a Staff Nurse in both the community and acute hospital setting for the past 8 years. Her most recent role was working on the Stroke Unit at Williton Community Hospital.

Melissa supports patients with the management of respiratory disease .

Nurse Alexandra Cunningham (f)

Alex joined the team in December 2016.  She has a special interest in cardiology having worked for a number of years within the cardiology department at Musgrove Park Hospital.  

Alex has recently completed her asthma diploma and now reviews patients with respiratory conditions.

Treatment Room Nurses

Mr Stewart Harding (m)
Nurse Callum Gardiner (m)

Callum joined the team in August 2017, having previously worked in the Emergency Department at Musgrove Park Hospital.

He is a member of the Duty Team who regularly provides Telephone Triage and assesses the needs of patients who request an urgent appointment, signposting them to the most appropriate member of the practice team.

Callum currently performs blood tests, ECGs, wound management, adult immunisation.

Healthcare Assistants

Nurse Dawn Carey (f)

Dawn undertakes blood tests, ECGs, blood pressure checks, wound care, pre-birthday reviews and flu vaccinations.  She also supports patient for weight management.

Dawn runs a regular Drop-In session at Bridgwater College as part of our Needadoc service.
